The ACMPR provides a legal framework for patients to legally use marijuana for beneficial relief of various symptoms. Our physician’s role is to assess patient eligibility through document review as well as patient interviews. All consultations are conducted either through Skype or FaceTime. Virtual consultations allow patients from across the country to benefit from our services that are otherwise unavailable.
Asking your health care practitioner for the above mentioned documentation is the simplest way to go about obtaining the documentation. We also have an Authorization for Medical Release form that we can send for you to sign and submit to your doctor.
Yes. If you are under the age of 25 you must have a parent or guardian sign a consent form and they must be present during your appointment. We understand you are above the age of majority, however long term cannabis use in patients under the age of 25 may be related to reducing neural development of the brain, which has not fully developed until the age of 25.
