To fully comply with Access to Cannabis for Medical Purposes Regulations (ACMPR), Green Relief has to receive and verify specific documents before registering and approving a patient.
To begin the process, fill out the application form (located here). Then mail the completed application to us at the address listed below, along with your fully completed and valid original medical document. Health Canada will only accept original mailed documents, so please do not email.

Alternatively, your doctor’s office may fax both the Medical Document and the Registration Application forms to us at 1-289-919-0382. You will be notified by email once you become approved, which typically takes 1-3 days depending on your doctor’s verification timeline.
Your pre-existing MMAR ATP, PUPL or DPPL can no longer be used to register with a licensed producer, as the validity dates have all passed. In order to register under the ACMPR, you will need to obtain a new medical document.
When your prescription expires, you will need to complete a new registration application and submit a new, original medical document. We will notify you well in advance of this happening. Please note that the expiration date is the final date that you can RECEIVE an order. As such, the final date for ordering will be three (3) business days prior to expiration.
Yes, you can be registered with more than one licensed producer at a time, as long as you are not providing the same medical document to both. In order to register with Green Relief, we require the original medical document completed by your prescribing doctor.
Your prescription is valid only during a specific time period provided by your physician, this can be up to, but no longer than a year.
Due to Health Canada regulations, we cannot give your original medical document back to you once you have become a registered client with Green Relief. We recommend that you take a photocopy of it for your own records.
Should you need to cancel your registration with Green Relief, you are free to do so without any fees or penalties. Unfortunately, the ACMPR states that once you have submitted your Medical Document to a licensed producer (LP) and you have been registered, you cannot take that document back and transfer it to another LP. You would need to cancel your relationship with your existing LP, get a new medical document from your healthcare practitioner and then register with your new preferred LP.
